These questions must be answered before filling out the General Application to see if you qualify.

General Application

The General Application along with short response questions much be filled out and submitted online. We recommend that you work on the short response questions separately before filling out the application.

Step 3

Healthcare Provider Form

This is NOT required, however, including this form in your application would increase your chance of receiving the grant. Please print the form for your healthcare provider to complete. Healthcare providers may scan the completed form and send it on your behalf to: rxcharity.contact@gmail.com.

Frequently Asked Questions

An individual must be 18 years or older to apply. For parents/guardians interested in applying for their child or other relatives, you may do so. Please indicate that you are applying for a minor or relative in the the application. RxCharity will provide prescription medication grants for those who demonstrate need. Some examples include: 

  • Recent unemployment
  • Economically disadvantaged/demonstrate financial need

RxCharity application grants are now open. Until we run out of funding, all are welcomed to apply. 

No, optional Healthcare Provider Form must be filled out by your physician that you are currently seeing.

We suggest that you look at your prescription costs and apply for the medication(s) that you think we can reasonably cover in the grant. Our program is meant for short-term relief and assistance to community members. As we work to secure more funding and expand, we hope to provide more resources to grant applicants.

We will not fund cannabis.

We ask that applicants please apply for one grant per cycle.
